Patients with chronic conditions are encouraged to attend an annual review to ensure they are on appropriate medication and their condition is well-managed.
Annual reviews cover a range of health conditions, including:
- Asthma and Chronic Obstructive Pulmonary Disease (COPD)
- Cardiovascular/Heart Disease, such as Atrial Fibrillation, Heart Failure, Chronic Heart Disease (CHD) and Hypertension
- Chronic Kidney Disease (CKD)
- Diabetes including foot check, bloods and blood pressure
- Dementia
- Learning Disabilities
- Mental Health Conditions
- Stroke and Transient Ischaemic Attacks (TIAs)
We encourage patients to take an active role in managing their health, but visiting the practice once a year for a review is beneficial.
Review Process
Most reviews are conducted by our nursing team. You may need two appointments, one with a Healthcare Assistant (HCA) and another with a Practice Nurse or Pharmacist. Patients will be invited to attend their annual review.
Some annual reviews can be conducted over the phone at the patients request and where appropriate.
During Your Appointment
The HCA will perform various tests and checks as needed, which may include:
- Measuring Body Mass Index (Height and Weight)
- Blood tests
- Blood pressure and pulse checks
- Cholesterol testing
- Diabetic foot examination
- Inhaler technique assessment
- Lifestyle and dietary advice
- Medication review
The HCA will inform you of any necessary follow-up, which may include an additional appointment with a practice nurse.
Preparing for Your Appointment
If you have diabetes, pre-diabetes, kidney disease, heart disease, or hypertension, please bring a urine sample. Specimen bottles are available at reception.
Attending your annual review is essential for proper management of your condition. If your review is overdue, we may not be able to re-authorise repeat prescriptions.
