Healthy Lifestyles service (Tier 2)
The Healthy Lifestyle service is available to families who would like support and or guidance around healthy eating and increasing physical activity. Offering confidential and personalised advice over a period of 12 weeks to help you make healthy lifestyle choices.
– Age 16 and above Face to face and phone support
– Online sessions
– Support for up to 12 weeks
– Follow ups 12 months post sign off

Services Available By Referral From Your GP Team
Health And Wellbeing Coach (Tier 2)
Health and wellbeing coaches support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them.
Health and wellbeing coaches can be an effective intervention for people experiencing a range of long term conditions, including respiratory, cardiovascular (including type 2 diabetes and hypertension), and stress/low mood. They can also support people with weight management, diet and increasing activity levels.

Digital Weight Management Programme (Tier 2)
The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.
It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access.
Who Can Be Referred To The Programme?
- You must be 18 or over.
2. You must have a BMI greater than 30. The BMI threshold will be lowered to 27.5 for people from black, Asian, and ethnic minority backgrounds, as we know people from these ethnic backgrounds are at an increased risk of conditions such as Type 2 diabetes at a lower BMI.
3. You must have diabetes, high blood pressure, or both.
4. You must have a smartphone, tablet, or computer with internet access.
5. If you would like to be referred to digital weight management please complete an online form or speak to a clinician at your appointment or speak to your local pharmacist.
Specialist Weight Management Services And Weight Loss Injections
Weight loss injections are currently only available through specialist weight management services, not directly from your GP.
Your GP can refer you to a specialist weight management service if they believe you are eligible for treatment with these medications.
The National Institute for Health and Care Excellence (NICE) recommends that these medications be used within specialist weight management services, and not by GPs.
The NHS is implementing the NICE guidance in a phased manner, meaning it will take time for the treatment to become widely available.
You can also explore private weight management clinics that offer these injections, but you will need to pay for the treatment yourself. Unfortunately GPs are not able to offer advice on starting these treatments or managing any side effects. We are not able to take over prescribing or monitoring private treatments on the NHS.
Referral to Specialist Weight Management Services (Tier 3) Your GP can refer you to specialist weight management services in Gloucestershire Hospitals NHS Foundation Trust or Worcestershire Acute Hospitals Trust if you met the criteria.
To Be Referred You Must Have Completed, (in the preceding 12 months prior to referral)
1. A Tier 2 or equivalent programme AND subsequently 6 months self-management AND
2. Have a BMI: 40+
OR Have a BMI >35 and at least 1 significant weight-related health condition e.g. diabetes
OR Have a BMI 30 to 34.9 and meet additional NICE criteria
(For patients of an ethnic minority background, BMI thresholds should be reduced by 2.5 kg/m2)
